背景:老年衰弱對於即將進入超高齡社會的台灣而言是一個重要的議題。許多慢性病如慢性腎臟病(Chronic Kidney Disease, CKD)都會加重老年衰弱的程度。台灣65歲以上之老年人口約有四成罹患有慢性腎臟病,如果能及早有效掌握透析前之衰弱進程與相關因素、辨識出衰弱高危險群並加以介入,為老年CKD患者規畫適當的健康促進與延緩失能的對策,便有機會逆轉衰弱的狀態。方法:本研究以某南台灣醫學中心尚未開始進入透析階段之門診高齡CKD患者為對象,探討目前老年CKD個案衰弱現況與相關之生理與功能表現並探討此族群發生衰弱之相關風險因子。研究中除了記錄CKD受試者例行門診檢查之生理數據之外,CKD患

者之上下肢重點肌力與手握力、五次起坐時間(Five Times Sit-to-Stand Test, FTSST)、三公尺計時起走測試(3m Timed Up-and-Go Test, TUGT)等,肌力與行動平衡評估均以標準量化方式進行測量。此外,受試者之日常生活功能、營養、憂鬱、認知等狀態,分別使用巴氏量表(Barthel Index, BI)、簡式迷你營養評估(Mini-Nutritional Assessment Short Form, MNA-SF)、十五題版老年憂鬱量表(Geriatric Depression Scale-15, GDS-15)、極早期失智症篩檢量表(Ascert

ain Dementia 8, AD-8)以面談方式進行評估。受試者亦接受肌少症篩檢(SARC-CalF)並以臨床衰弱量表(Clinical Frailty Scale, CFS)進行衰弱與非衰弱之分類。結果:根據本研究所完成之191位門診CKD個案評估(平均年齡76.41±6.47歲;女性99人),其中有109位屬衰弱族群(CFS≧4),其年齡顯著高於非衰弱族群,腎臟病分期也有較高比例偏向晚期(3b-5期),血清尿素氮超過標準值者與血鈣較標準值低者均較多。衰弱組之肌少症與極早期失智症風險人數比例與得分顯著較多也較高,憂鬱得分顯著較高,以及營養不良得分顯著較低。功能性評估亦顯示衰弱組之老年CK

D個案其手握力、肱二頭肌、股四頭肌肌力均明顯低於非衰弱組,五次起坐時間與TUGT之時間也明顯較長。若以TUGT時間作為跌倒風險判定,可發現衰弱組無論男、女都屬跌倒之高危險群。年齡較大的女性衰弱族群其握力、肱二頭肌肌力明顯低於年齡較輕者(老老組<中老組<初老組),而男性衰弱族群其功能性評估於不同年齡間則無明顯差異。複回歸分析顯示與衰弱呈正相關的風險因子為年齡、TUGT之時間、以及腎臟病晚期(3b-5期),負相關的風險因子為肱二頭肌肌力、白蛋白(Albumin, Alb)。結論:本研究之老年CKD個案衰弱比例為57.07%。衰弱組之特性為年齡較大、腎功能較差、有較高的比例有肌少症風險、較高的肌少症

風險得分、較高的憂鬱得分、較低的營養不良得分、極早期失智症人數比例較多、得分也較高,以及功能性評估較差。年齡、肱二頭肌肌力、三公尺計時起走、腎臟病晚期(3b-5)及Alb是老年CKD個案衰弱的風險因子。建議未來可將衰弱評估納入慢性腎臟病個案固定追蹤項目及臨床照護指標,並運用跨團隊合作及結合社區健康促進資源來延緩衰弱與失能、提升老年生活品質。

背景與動機:台灣已邁入高齡社會,長照十年計畫2.0的社區整體照顧服務,經由照顧管理的評估機制,將服務輸送給符合資格的長者接受復能照護服務,而本研究欲深入了解復能服務的效果。目的: 居家復能服務個案之接受服務前後之日常生活功能成效。研究方法: 使用次級資料庫進行回溯式研究設計(retrospective study),取自衛生福利部照顧管理資訊平台,107年1月至108年1月臺北市長期照顧管理中心之訪視新案,並接受照顧管理評估量表初評,確定個案長照需要等級(CMS)符合使用長照服務資格,並且接受復能服務一年。在資料庫當中,紀錄共計69138筆資料,經編號整理過後,符合資料2357位個案。依「照

顧管理評估量表」之社會人口學、日常生活功能評估等進行資料分,並以多因子變異數分析與廣義估計式(Generalized Estimating Equations, GEE)進行統計分析。研究結果:本研究之個案共計2357人,平均年齡為82.2歲,女性佔60.5%,已婚佔45.8%,並有80.9%為與家人同住,罹患慢性病的佔比,高血壓61.9%、糖尿病28.9%、冠心疾病27.8%、關節炎23.9%、失智症23.0%、腎衰竭21.3%、腦血管意外19.8%、癌症11.4%、精神疾病10.4%。而經過復能後三次評估,在CMS等級第三級以及第四級於ADL總分呈現退步趨勢,但在第五級、第七級、第八級於總

分上則有進步,第六級則持續維持原有之身體功能,復能訓練之介入六個月有成效。在全體總分來看,第一次後測及第二次的後測皆退步。而在經過復能訓練之後,發現到罹患糖尿病、腦血管意外及關節炎之個案會影響到訓練之成效(退步),另罹患冠心疾病及癌症之個案有機會在復能訓練中獲得較好的成效。研究結論: 居家復能服務以3個月或12次短期、密集的介入,在日常生活功能上看到成效。個案接受復能訓練後在日常生活功能的成效,隨著介入三個月、六個月後,在吃飯、穿脫衣服、移位、走路及上下樓梯等分項能有顯著的進步。然而,日常生活功能方面,年齡越大、男性、有糖尿病、關節炎、腦血管意外、與他人一起居住、CMS的等級越高,皆會使得日常

生活功能量表的分數越低。
