目的:良性陣發性眩暈(BPPV)是臨床上最常見的週邊性眩暈,在以往的統計中,多數認為與陳舊性頭部創傷、內耳或中耳發炎性疾患、代謝或退化性疾病有關,然而最近的文獻顯示地震發生以後和眩暈症發作的就診率存在有一定程度的相關,所以本項研究希望從地震發生前後因良性陣發性眩暈前來就診的病患資料中,探討地震數據是否與就診數、不同年齡和性別族群有關連,如果關聯性存在的話作者將尋找其可能的解釋和迴歸方程式並提出實際應用上的建議。方法:本項研究的資料來源是作者執業之耳鼻喉專科診所從2019年3月到2021年3月間的就診病患去識別化資料,關聯資料來自中央氣象局地震測報中心,採用回溯性分析比較良性陣發性眩暈和地震的

關聯性,以SPSS 第27版作為統計分析工具,分析良性陣發性眩暈患者族群當中,地震事件是否對不同性別、年齡、慢性病族群有不同影響,並分析地震深度、震央規模和離震央距離是否與良性陣發性眩暈有關聯性;為了瞭解同一時期其他耳科疾患前來就診的患者是否受到地震因素影響,作者收集同一時期的急性外耳炎患者作為對照組進行統計比較。結果: 統計採95%信賴區間,兩週間因BPPV前來就診的病例數與兩週後台南地區四級以上有感地震的 Pearson 相關係數為 0.321,「二週間BPPV病例數」與「後28天內台南地區四級以上有感地震」的 Pearson 相關係數為 0.355,推論一元 Logistic 迴

歸方程為: 「後28天內有感地震數 」= 1.003 + 0.154 *「二週間BPPV病例數」,模型擬合度 = 0.109;比較同一時期因急性外耳炎前來就醫的病例數與地震發生數並無顯著相關性;根據迴歸方程,將不同時間週期BPPV病患數所預測之地震數與實際地震數對比,實際發生數大於預測數定義為高關聯組,實際發生數低於預測數定義為低關聯組,採計95%信賴區間,發現關聯性的高低與地震的震央規模、距離、深度、性別、年齡、慢性病、好發半規管皆不存在顯著關聯性。結論:地震與良性陣發性眩暈的關係存在有趣的因果關係,似乎在有感地震尚未造成地層錯動之前,已然有影響人類內耳耳石穩定性的現象發生,至於其確切關聯性

和機轉則有待更進一步的研究。
